Forms are mandatory and must be completed prior to tryouts:

All athletes must complete all forms found in the parent portal at RankOneSport:

• AISD Participation Forms (REQUIRED)​​

(These include Physical, Medical History, UIL Forms, and Contact Forms)

• AISD Physical Form (2023-2024) (REQUIRED)

• Private Transportation Permit (REQUIRED)

• Emergency Student Information Card (REQUIRED)
