• AISD Participation Forms (REQUIRED)​​

(These include Physical, Medical History, UIL Forms, and Contact Forms)

• AISD Physical Form (2024-2025) (REQUIRED)

• Private Transportation Permit (REQUIRED)

• Emergency Student Information Card (REQUIRED)
